2024 Community Resource Guide now available

LIBERTY — Sullivan 180, in partnership with Sullivan County Public Health and the Rural Health Network, are excited to announce the release of the third edition of the Community Resource Guide. The Guide is a compilation of resources, services, and organizations available to Sullivan County residents and the immediate area. The Guide can assist with identifying support for needs ranging from finding healthcare providers, food pantries, housing, mental health support, to services for individuals with disabilities and much more.

According to the Community Health Needs Assessment, Sullivan County residents stated “lack of knowledge of existing resources” as their #1 barrier to improving their own health. The Resource Guide is a step toward breaking down that barrier as we work to improve our health outcomes. 

New to the Guide this year is a Parks and Public Spaces category where you can find a local park or trail near you to be physically active or reconnect with nature. The Guide also now notes participants in the Unite Us referral platform, which facilitates connections for anyone in need by being a direct referral and link to government, healthcare, and local community-based organizations offering a variety of services.  

You can find Community Resource Guides in key locations throughout the county including town halls, schools, libraries, and agencies.
