Jeffery M. Jacobson of Callicoon Center, NY a Licensed Practical Nurse passed away on Sunday, February 23, 2020 after a nine year battle with heart failure. He was 49. The son of Donna L. Wood Susi and the late Jon F. Jacobson he was born January 17, 1971 in Albany, NY. Jeffery was a 1990 graduate of South Colonie High School and a 1997 graduate of Sullivan BOCES Practical Nursing program. He was married to Melissa A. Mootz in 1997.

Jeff is survived by his wife Melissa A. Mootz Jacobson at home, his five children: Matthew J. Bounds of Midlothian, VA, Timothy J. Jacobson of Liberty, NY, Leah M. Jacobson, Sarah A. Jacobson and Abigail L. Jacobson all at home. One grandchild Jordan Bounds of Midlothian, VA, his mother Donna L. Wood, Susi and her husband Frank of Saratoga Springs, NY, his extended family Jack and Cindy Mootz of Callicoon Center, NY, Chris and Greg Hawkins, Morgan Hawkins, Erika Stauch (Steve), Brian Stauch, and Thomas Stauch of North Branch, NY, Jackie Mootz and Adam Carlsen of Livingston Manor, NY and Stephen Mootz of Callicoon Center, NY. Also his special “niece” Aubree Harasemovech and special friends: Justin & Jessica Angerstein of Novi, MI, Jeffery Weinstein of Rock Hill, NY, Scott McBride of Liberty, NY and Thomas & Lisa Moran of Jeffersonville, NY.

In addition to his father he was predeceased by his brother Ronald Jacobson and his grandparents Orville & Ingrid Wood and Leah & Frederick Jacobson.

Jeff was the epitome of of what a loving husband , father and friend should be. We will never be alone as his love for us all is infinite.

Jeff loved to have fun, and laugh. His sense of humor was infectious. He possessed a natural skill for making you laugh or smile with his practical joking skills, sarcasm and was able to find humor in almost every situation. Jeff loved to share a beer, a meal, a sweet dessert and a good laugh.

Please join us for a celebration of his life on Saturday, March 7th at the Youngsville Firehouse from 4 PM to 7 PM, for food, drink and memories. In true Jeff fashion we ask that you bring a picture, a story, or a memory of Jeff to share with us all. We will be releasing paper lanterns after a short memorial service around 7 PM.

We look forward to seeing you all and thank you for helping us to say goodbye to Jeff, a loving husband, father, grandfather, son, brother-in-law and friend.
